1099 MISC Withholding Exemption Certificate
(Tax withholding is required on certain payments of Pennsylvania source compensation, business income and lease payments made to nonresidents)
Purpose of this Form: To Determine the residency status of individuals, estates or trusts for to identify the type of entity for determining whether Pennslyvaian personal income tax withholding is required on payments of non-employee compensation or lease payments, such as rents, royalties, bonus payments, damage payments, delay rents, or other payments pursuant to a lease.

IRS Request for Taxpayer Identification Number and Certification
An individual or entity (Form W-9 requester) who is required to file an information return with the IRS must obtain your correct taxpayer identification number (TIN) which may be your social security number (SSN), individual taxpayer identification number (ITIN), adoption taxpayer identification number (ATIN), or employer identification number (EIN), to report on an information return the amount paid to you, or other amount reportable on an information return.

Honorarium Agreement/Supplier Information Form
Purpose of this Form: To document the intent to provide a gratuitous payment of money to an individual as a show of appreciation for
their participation in a usual academic activity for which no fee is legally required and/or to establish the individual as a supplier (as
applicable).

Purpose of the Form: To provide information for a new supplier or update information for existing supplier. For our purposes the Term Supplier can be an individual/sole proprietor or business
